BoneMRI

by MRIguidance

AI Application
Low Confidence

BoneMRI is image-processing software that turns a dedicated MRI scan into CT-like images of bone, so that radiologists and orthopaedic surgeons can assess bone shape without the X-ray dose of a CT scan. The instructions for use cover the spine (cervical, thoracic, lumbar and S1) and the pelvic region in patients aged 12 and older, to assist diagnosis, monitoring and treatment planning of orthopaedic conditions; it also produces synthetic T1-weighted images. The vendor says a pre-trained encoder-decoder neural network, trained against CT scans of the same patients, reconstructs Hounsfield-unit values voxel by voxel; it does not learn from the hospital's data and does not report or diagnose. The imaging expert checks image quality and reads the BoneMRI images with the full MRI exam, never on their own. A gateway in the hospital pseudonymises the MRI data and sends it to a cloud service run by MRIguidance, which returns the images to the PACS as an extra series; the vendor names no cloud provider or region.

Instructions for Use
Notified Body: Kiwa Assurance B.V.(Cert: 23M00065CRT01)
The BoneMRI instructions for use (v1.12.1, 2 Oct 2026; https://mriguidance.com/wp-content/uploads/2026/10/IfU_BoneMRI_V1.12.1_EU_EN_20261002_print.pdf, read 2026-10-20) state that BoneMRI is not to be used in acute settings because of the acquisition and reconstruction time (maximum reconstruction time 90 minutes for the pelvic region and 20 minutes for the spine), is not to be used for diagnosis or monitoring of primary or metastatic tumours, is indicated for patients aged 12 and older and only for the spine and pelvic region, requires the dedicated BoneMRI scan protocol acquired before contrast agents, and that image assessment must never rely solely on BoneMRI or synthetic T1w images. Imperfections in the source MRI carry into the output, patient motion can cause misalignment in stitched images, and HU measurements should be made on the original, not reformatted, images. The gateway and cloud are not for long-term storage.
